At Mt. Horeb, we believe the movement of God in the world beckons us to participate! Thank you for your willingness to answer the call and join a team.

You can find basic details about each upcoming trip, including dates, cost, deadlines, and Team Leader contact information, at mthorebchurch.org/missions/joinateam. If you have questions about any portion of this application, please contact the Team Leader.

In order to be considered for any missional team, local or global, individuals must fill out the following application and submit a non-refundable deposit. If the participant is under the age of 18, a legal guardian will have to review and sign as well.

Please fill out this application completely. The non-refundable deposit for international mission trips or trips involving airline travel is $250 and must be paid with the submission of this application. For domestic trips, the deposit is $100.

The Team Leader will notify the applicant once they have been accepted to the team.

NOTE: At the end of this application you will find the Release and Waiver of Liability and the COVID-19 Assumption of Risk and Release of Liability. If you would like a physical copy of these forms, please request them from the Team Leader.

Rising 9th and rising 10th graders are invited to join our Alaska mission trip, serving with Next Step Ministries in Fairbanks! Trevor Miller will lead the team on an epic adventure in ministry to our northern-most state, supporting and continuing in construction of a spiritual retreat center.

Rising 11th through Graduated Seniors are invited to join our Costa Rica mission trip, serving our missionary partners Daniel and Sarah Barrett! Lexington was the Barrett's home before entering the mission field to build and operate a church and ministries in Jaco, a town along Costa Rica's Pacific coast.

Passport Copy

If you are applying for an international trip, you'll need to have a valid passport before airline tickets are purchased. Your passport must have six-month validity from the last date of the trip. For example, if the team returns on January 31, your passport much have an expiration date that is no sooner than July 31 of that year.

By typing my name below, I certify that all information on this application is accurate and complete. If I were to be approved as a team member, I am also giving consent for video/photos/audio that include my name, likeness, or voice taken by representatives of Mt. Horeb Church to be used for promotional purposes within the church, on printed and electronic media, and on electronic platforms, including but not limited to Mt. Horeb's website, official social media accounts, and digital media. In addition, I am agreeing to treatment by medical staff if an emergency arises while on this trip.

Release and Waiver of Liability

This is a copy of the Release and Waiver of Liability required to travel with Mt. Horeb. You may also be asked to sign a physical copy of this form prior to departure. By typing my name below, I recognize that there are risks involved in participating in the mission trip and hereby assume all risk of injury, harm, damage, or death in connection with my participation in it. I hereby acknowledge that by engaging in this mission, I am subjecting myself to certain risks voluntarily, including and in addition to those risks which I normally face in my personal and business life, including but not limited to such things as health hazards due to poor food and water, diseases, pests, and poor sanitation; potential danger from lack of control over local population; potential injury while working; and inadequate medical facilities, etc. I understand and agree that neither Mt. Horeb nor its trustees, officers, directors, employees, agents or representatives may be held liable in any way for any injury, harm, damage, or death that may occur to me as a result of my participation in this mission trip and hereby release Mt. Horeb, its trustees, officers, directors, employees, agents and representatives from any injury, harm, damage or death, which may occur while I am participating in the mission trip. To the fullest extent permitted by law, I agree to save and hold harmless Mt. Horeb, its trustees, officers, directors, employees, agents and representatives from any claim by myself, my estate, heirs, successors, assigns or other persons arising out of my participation in the mission trip. Further, I hereby release and discharge the mission organizations which assisted in these arrangements, their agents, employees, and officers, from all claims, demands, actions, judgments, and executions which I ever had, or now have, or may have or claim to have, against the mission organizations, their agents, employees, and officers, and their successors or assigns for all personal injuries to personal property, real or personal, caused by, or arising out of, the above described mission service. If I am unable to do so, I authorize Mt. Horeb through its trustees, officers, directors, employees, agents or representatives to consent to any necessary examination, anesthetic, medical diagnosis, surgery, or treatment and/or hospital care rendered to me under the general or special supervision and on the advice of any physician and surgeon licensed to practice medicine by the state or country in which they practice, during the duration of the trip.

COVID-19 Assumption of Risk and Release of Liability

This is a copy of the COVID-19 Assumption of Risk and Release of Liability required to travel with Mt. Horeb. You may also be asked to sign a physical copy of this form prior to departure. The 2019 Novel Coronavirus (“COVID-19”) is a known and rapidly evolving pandemic that is affecting travel worldwide, with continued spread and impacts expected. I am fully aware of the current global COVID-19 virus outbreak, the current travel restrictions, and inherent risks involved when choosing to travel. I am aware that it is my personal decision to travel and am doing so with full knowledge of travel recommendations, restrictions, regulations, and risks with regards to COVID-19, as well as the possibility of future travel restrictions, and take full responsibility for my actions with regards to traveling. I understand and agree that participating, including travel, in this mission trip may result in a higher risk of contracting COVID-19. I understand and agree that both the known and potential dangers of traveling and acknowledges that traveling may, despite reasonable efforts to mitigate such dangers, result in exposure to COVID-19, which could result in quarantine requirements, COVID-19 testing and associated costs, serious illness, disability, and/or death. I assume full responsibility for and risk of illness, bodily injury, death, or property damage to myself due to negligence, active or passive, or otherwise while on this mission trip. I understand and agree that neither Mt. Horeb nor its trustees, officers, directors, employees, agents or representatives may be held liable in any way for all consequences of participating in this mission trip related to COVID-19, including but not limited to: travel warnings, travel restrictions, travel rules, increased risk of exposure or contraction, illness, death, property damage, financial loss, costs, penalties, fees, fines, cancellations, postponements, quarantines, health regulations and/or screenings, regardless of whether these consequences are imposed by governmental agencies of the locations to and through which I am traveling and/or related organizations or entities, including but not limited to: airlines, airports, hotels, missions partners, transfer agencies, and/or any other provider involved in this trip. I further agree not to institute a credit card dispute or “charge back” to Mt. Horeb for any of these reasons. I understand that although Mt. Horeb has some level of insurance for mission trips, travel insurance generally only covers unforeseen events. Most insurers now classify COVID-19 as a 'known event' and will not cover COVID-19 related cancellations, delays, quarantines, or additional costs or fees. Most policies also have a specific clause stating they do not cover epidemics and pandemics, especially when travel warnings are in place. Additionally, I understand that my concerns or fear of travel is not a covered reason for cancellation and will not be covered by Mt. Horeb or their travel insurance. I have carefully read and voluntarily sign this Assumption of Risk and Release of Liability related to COVID-19.

COVID-19 Guidelines

Mt. Horeb will continue to monitor and adhere to current CDC guidelines regarding all aspects of the COVID-19 pandemic, including travel requirements and restrictions. As the departure of this trip approaches, the Team Leader will be in conversation with the team as it pertains to updated CDC guidelines and how that might affect team members.
